What are Care Navigators?

Care Navigators are trained members of the practice team who help you access the right support at the right time. They can:

  • Direct you to the most appropriate member of the healthcare team for medical concerns.

  • Connect you with other services, such as employment, financial advice, or support for carers.

Please note: Care Navigators do not provide clinical advice. Any medical concerns will be referred to the relevant healthcare professional.

Benefits for Patients

Using a Care Navigator can help you:

  • See the right professional the first time, potentially reducing waiting times.

  • Understand your options and make informed choices about your care.

  • Access care faster and more efficiently.

What Happens When I Speak to a Care Navigator?

When you speak with a Care Navigator, they will:

  1. Ask basic questions about your health concern or query.

  2. Identify the most appropriate source of care, whether within the practice or in local support services.

All conversations are confidential, and staff are trained to uphold the same principles of privacy as the rest of the practice team.

Why Does the Receptionist Ask About My Problem?

Reception staff ask about your reason for visiting to ensure:

  • You receive care from the right health professional at the right time.

  • Doctors can prioritise house visits and phone calls appropriately.

  • You are directed to a nurse or another professional when suitable.

Receptionists are trained and bound by confidentiality. Any information you provide is treated strictly confidentially. You can request to speak to a receptionist in private, and if you prefer not to disclose details, this will be respected.
