Instead of phoning the practice directly, all non-urgent medical or administrative requests can now be submitted using our online triage service, Patient Triage.

You can complete the form on a smartphone, through our practice website, or via the NHS App. The form allows you to provide details about your symptoms or administrative request quickly and securely.

Once submitted, your request will be reviewed by the most appropriate member of the team, who will determine the next steps and how best to help you.

There is no need to download an app or create an account to use the service.

To help us process your request efficiently, please provide as much clear and concise information as possible when completing the form.

  • Select either a medical or administrative request
  • Confirm the request is non-urgent
  • Complete the form with as much information as possible
  • Tell us how you would like to be contacted
  • Enter your details and submit the form

If you do not have access to the internet or find it difficult to use a smartphone or computer, please contact reception by telephone and Care Navigators will be happy to help you complete the Patient Triage form if needed.
